Download a template Letter of Authorisation
Should a patient wish to be represented by someone else, we need this confirmed by a letter of authorisation. The person who has been duly authorised will then be able to sign the injury report form.

If you are unable to open the above documents in Word or Acrobat Reader, try saving them to your hard disk, then try to open them from there.
To open the document:
1. Click on the desired form with the right mouse button. A menu will appear on the screen.
2. Select "Save target as..."
3. Find an appropriate folder for saving the form. Select "Save".
4. Start Word / Acrobat Reader on your computer.
5. Try to open the document on your own computer.
