You can make an application for compensation for adverse reactions to drugs. Additionally, you can apply for compensation for treatment failure. The claim form can be found here.

Adverse reactions and treatment failure
Treatment failure may include incorrect dosing and incorrect drugs. If you want to apply for compensation for both adverse reactions to drugs and treatment failure, you must complete NPE claim form.
Recommended vaccines
If you are applying for compensation after receiving a vaccination as part of the National Vaccination Programme, you need to apply using the patient injury claim form.
Drug injury
If you want to apply for compensation for adverse reactions to drugs, you must complete the drug claim form.
