Forms
You can sign and submit an application for compensation digitally. You will find guidance on how to do it inside the digital form. All other forms must be printed, signed and mailed. You can also submit the forms through My page.
When filling in a form, we recommend that you use a modern browser such as Google Chrome, Microsoft Edge, Apple Safari or Mozilla Firefox. In addition, we recommend that you install the PDF reader Adobe Acrobat Reader, which is free.
Once you have completed the form, please save it locally on your computer before sending it to us. Open the form and check that all the content has been saved. In case of problems, contact our service center.
